Education and Nutrition Benefits Form for meals and more!
Although families won’t have to pay for meals, they must still complete qualification forms for their local school. Completed forms allow schools to qualify for meal REIMBURSEMENT from the state as well as funding for;

Instructional specialist staff (ex. Reading Interventionists, Math Tutors, Academic & Behavior Aids)
Teaching supplies and materials
Counselors and Social Workers
School Nurses
Professional Learning for staff
Parent and Community engagement supplies and activities
Technology for students

Please take a moment to complete the Education and Nutrition Benefits form and return it to your student’s school or to APS Food Service at 2373 Gordon Road; Alpena, MI 49707.
